Participants Release and Waiver of All Liability

   I, the undersigned being the parent/guardian, in consideration of my child being permitted to participate in McLoud Youth Football and Cheer programs and activities do hereby release, waiver, and forever discharge the McLoud Youth Football and Cheer, McLoud School System, its agents, officials, employees, or other participants from any claim, demand, action, or right of action arising from or by reason of known or unknown property loss, damage, or personal injury resulting from any incident which may occur as a result of participation in such activity whether caused by the acts or in the actio of those released beyond what our present insurance covers. Further, I understand that participating in cheer, football and other such activities has inherent dangers and that my child is in acceptable physical condition to participate in this sport, that I have fully disclosed all known medical conditions that would affect said participation. I further state that I have read that foregoing Release and Waiver of All Liablity and known the content therefore and sign as my own free agent.
